Caregiver Coach

Full-Time · Carmel, IN & surrounding areas

Apply

Provides individualized coaching, education, and ongoing support to lay caregivers participating in the Structured Family Caregiving (SFC) program. Equips caregivers with skills and strategies to manage waiver participants' chronic medical conditions and behavioral health needs related to cognitive impairment and/or dementia.

Responsibilities:

  • Complete accurate and timely documentation of caregiver assessments, coaching sessions, care plan updates, and communications
  • Maintain compliance with HIPAA and agency confidentiality policies
  • Conduct initial structured caregiver assessments to identify goals, values, strengths, and support needs
  • Provide ongoing caregiver coaching in-person, virtually, telephonically, or through HIPAA-secure platforms
  • Engage with caregivers biweekly to provide education, tools, and strategies
  • Coach caregivers on managing chronic conditions, dementia-related behaviors, communication strategies, and home safety
  • Monitor caregiver adherence to approved care plans
  • Assist in developing and updating written crisis management and emergency plans monthly
  • Collaborate with Registered Nurses, care managers, and service coordinators
  • Refer medical or skilled nursing concerns to appropriate licensed healthcare professionals

Requirements:

  • Bachelor's degree in Social Work, Nursing, Human Services, or related field (or equivalent combination of education and experience)
  • Minimum 2 years experience working with elderly individuals, individuals with disabilities, or caregivers in a coaching, case management, or support role
  • Demonstrated experience working with lay caregivers required
  • Knowledge of Indiana Medicaid waiver programs, including Structured Family Caregiving and PathWays for Aging
  • Understanding of dementia, cognitive impairment, chronic medical conditions, and behavioral health needs
  • Strong interpersonal, coaching, and communication skills
  • Ability to conduct structured assessments and document services accurately
  • Ability to work independently while collaborating within an interdisciplinary team
  • Valid driver's license and reliable transportation
  • Pass a background check
